Title : Risk of developing mesothelioma due to neighborhood exposure to asbestos

Abstract : Routes of asbestos exposure consist of occupational and non-occupational exposures, and furthermore the later is classified as para-occupational, neighborhood or true general environmental exposure. Consequently, in order to evaluate health risk caused by neighborhood exposure to asbestos, it is necessary to exclude risk due to the other exposure routes from overall risk. We reviewed epidemiological studies on the relationship between neighborhood asbestos exposure and risk of mesothelioma. In studies on a crocidolite mine in South Africa and a chrysotile mine in Canada, occupational exposure was not excluded. In studies on a crocidolite mine in Australia and an asbestos manufacturing factory in U. S. A., risk caused by non-occupational exposure was evaluated, but the risk was not classified as para-occupational and neighborhood exposures. In a study on an asbestos factory in Italy, first, occupational and para-occupational exposures were excluded, and next, the incidence rate of mesothelioma in neighborhood residents was calculated, so that risk caused by neighborhood exposure couls be evaluated. In case-control studies in Italy, South Africa three European countries and the U. K., risks caused by occupational, para-occupational and neighborhood exposures were evaluated separately. As a whole, relative risk (RR) of neighborhood exposure in crocidolite and amosite mines was about 10 to30 and RR in major asbestos factories was about 5 to 20. On the other hand, statistically significant RR of neighborhood exposure was not observed in chrysotile mines and some asbestos facilities.

Abstract: The use in the past, and to a lesser extent today, of chrysotile asbestos in automobile brake systems causes health concerns among professional mechanics. Therefore, we conducted four separate tests in order to evaluate an auto mechanic's exposure to airborne asbestos fibers while performing routine brake maintenance. Four nearly identical automobiles from the 1960s have four wheel drum brakes were used. Each automobile was fitted with new replacement asbestos-containing brake shoes and then driven over a predetermined public road course for about 2253 km. Then, each car was separately brought into a repair facility; the brakes removed and replaced with new asbestos-containing shoes. The test conditions, methods, and tools were as commonly used during the 1960s. The mechanic was experienced in brake maintenance, having worked in the automobile repair profession beginning in the 1960s. Effects of three independent variables, e.g., filing, sanding, and arc grinding of the replacement brake shoe elements, were tested. Personal and area air samples were collected and analyzed for the presence of fibers, asbestos fibers, total dust and respirable dust. The results indicated a presence in the air of only chrysotile asbestos and an absence of other types of asbestos. Airborne chrysotile fiber exposures for each test remained below currently applicable limit of 0.1 fiber/ml (eight-hour time-weighted average).

Abstract: Data exist that show that chrysotile asbestos does not retain its mineral properties, or biological activity, at temperatures far below the olivine transformation point. Temperatures hundred of degrees below this point cause the mineral to lose structural water with accompanying crystal structure degradation. The loss of structure is accompanied by modification of its surface and reduction or loss of biological activity. Using heating studies and milling as an approximation of thermal and mechanical shear stress that chrysotile is subjected to on a brake lining, biological blunting is shown to begin much earlier than the olivine transformation process. Minimal degradation of the chrysotile surface structure imparts a disproportionately great effect on its biological activity. Biological and epidemiological data for brake workers exposed to chrysotile asbestos should be viewed in context with the conditions of service to which the product was subjected over a lower range of temperatures than previously considered important. Abstract: Objectives: A large number of workers in the USA are exposed to chrysotile asbestos through brake repair, yet only a few cases of malignant mesothelioma (MM) have been described in this population. Epidemilogic and industrial hygiene studies have failed to demonstrate an increased risk of MM in brake workers. We present our experience of MM in individuals whose only known asbestos exposure was to brake dust and correlate these findings with lung asbestos fiber burdens.
Methods: Consultation riles of one of the authors were reviewed for cases of MM in which brake dust was the only known asbestos exposure. Lung fiber analyses were performed using scanning electron microscopy (SEM) in all cases for which formalin-fixed or paraffin-embedded lung tissue was available.
Results: Ten cases of MM in brake dust-exposure individuals were males aged 51-73 yr. Nine cases arose in the pleura and one in the peritoneum. Although the median lung asbestos body count (19 AB/g) is at our upper limit of normal (range 0-20 AB/g), half of the cases had levels within our normal range. In every case with elevated asbestos fiber levels by SEM, excess commercial amphibole fibers were also detected. Elevated levels of chrysotile and non-commercial amphibole fibers were detected only in cases that also had increased commercial amphibole fibers.
Conclusions: Brake dust contains exceedingly low levels of respirable chrysotile, much of which consists of short fibers subject to rapid pulmonary clearance. Elevated lung levels of commercial amphiboles in some brake workers suggest that unrecognized exposure to these fibers plays a critical role in the development of MM.

Abstract : Chrysotile asbestos is often included with other asbestos materials in evaluation and classification. However, chrysotile is a serpentine with markedly different physical and chemical characteristics in comparison to amphiboles (e.g. crocidolite, amosite, tremolite). In contrast to amphiboles which are solid rod-like fibers, chrysotile is composed like a rope of many fine fibrils which tend to unwind.
In order to quantify the dynamics and rate by which chrysotile is removed from the lung, the biopersistence of a sample of commercial chrysotile from the Eastern Townships area of Quebec, Canada, labeled "QS Grade 3-F" which is the longest commercial grade intended for textile use was studied. As the long fibers have been shown to have the greatest potential for pathogenicity, the chrysotile samples were specifically chosen to have more than 200 fibers/cm3 longer than 20 um present in the exposure aerosol. This publication presents the results of this study through 3 months post exposure.
Study Design:
1) Fiber clearance (lung digestions): At 1-day, 2-days, 7-days, 14-days, 1-month, 3-months, and 12-months (to be reported) following a 5-day (6 hours/day) inhalation exposure, the lungs from groups of animals were digested by low temperature plasma ashing and subsequently analyzed by transmission electron microscopy for total chrysotile fibers number in the lungs and chrysotile fiber size (length and diameter) distribution in the lungs.
2) Fiber distribution (confocal microscopy): This procedure was included in order to identify where in the lung the fibers were located. At 1-day, 2-days, 7-days, 14-days, 1-month and 3-months (to be reported) post exposure, the lungs from groups of animals were analyzed by confocal microscopy to determine the anatomic fate, orientation and distribution of the retained chrysotile fibrils deposited on airways and in the parenchymal region.
Chrysotile was found to be rapidly removed from the lung. Fibers longer than 20 um were cleared with a T 1/12 = 16 days, most likely by dissolution into shorter fibers. The shorter fibers were also rapidly cleared from the lung with fibers 5-20 um clearing even faster (T 1/12 = 29.4 days) than those < 5 um in length. The fibers <5 um in length cleared at a rate (T 1/12 = 107 days) which is within the range of clearance of insoluble nuisance dusts. The breaking apart of the longer fibers would be expected to increase the short fiber pool and therefore could account for this difference in clearance rates. The short fibers were not found clumped together but appeared as separate, fine fibrils, occasionally unwound at one end. Short free fibers appeared in the corners of alveolar septa, and fibers or their fragments were found within alveolar macrophages. The same was true of fibers in lymphatics, as they appeared free or within phagocytic lymphocytes. Neutrophil-mediated inflammatory response did not occur in the presence of chrysotile fibers at the time points examined.
Taken in context with the scientific literature to date, this report provides new robust data which clearly supports the difference seen epidemiologically between chrysotile and amphibole asbestos.
